Comprehensive Reentry
At SCC we fully recognize that successful reentry
requires addressing the full spectrum of
an individual’s needs and sustained our
commitment to the holistic approach. In 2024,
SCC continued to prioritize care management
as a crucial tool in supporting our client
partners, ensuring they have the resources
and guidance to rebuild their lives.
Upon release, many individuals face
immediate challenges, including a
lack of vital documents, housing,
healthcare, and other basic
necessities. These obstacles can
quickly become overwhelming
and stand in the way of successful
reintegration. Without access to
essential items like identification,
Social Security cards, and birth
certificates, securing employment,
housing, or even basic services can
feel like insurmountable tasks.
SCC’s comprehensive care
management model takes a holistic
approach to reentry, addressing not
only the immediate crisis of document
recovery but also the long-term needs
of individuals. By connecting clients to
housing support, hot meals, behavioral
health services, substance abuse
counseling, and employment assistance,
SCC ensures they have the foundation
needed for a stable and independent life.
Our Care Managers work closely with each
individual, offering personalized support and
guidance to navigate the complexities of reentry
and providing the tools necessary to thrive. Through
this approach, SCC is helping individuals build a
brighter, more sustainable future, one step at a time.
